Membership Plan Details:

If the patient chooses to pay for treatment through a third-party financing company, the treatment discount will be reduced from 15% to 5% to cover the third party charges.

Adult membership cleaning must be scheduled once in 6 months not sooner.

Perio membership cleanings have to be scheduled once in 4 months. Patient's additional periodontal cleaning fee is eligible for 15% discount, if needed.

15% treatment discounts cannot be combined with any other in-house promotional offer(s).

The discounts will exclude Smile Perfected Whitening, any saliva tests, 4 on 1 dentures and all dental products.
