Membership Plan DetailsOpen
- If the patient chooses to pay for treatment through a third-party financing company, the treatment discount will be reduced from 15% to 5% to cover the third-party charges.
- Adult membership cleaning must be scheduled once in 6 months not sooner.
- Perio membership cleanings have to be scheduled once in 4 months. Patient’s additional periodontal cleaning fee is eligible for 15% discount, if needed.
- 15% treatment discounts cannot be combined with any other in-house promotional offer(s).
- The discounts will exclude Smile Perfected Whitening, any saliva tests, 4 on 1 dentures and all dental products.
- Membership plan cannot be combined with or used instead of active PPO dental insurance.
- Any cancellation due to new insurance plan would have to be evaluated on case-to-case basis for partial refund.
